Letter from Wish Coordinator

Hello!

My name is Rachel and I am the Wish Coordinator for A Special Wish Foundation’s Dayton Chapter. I am here for you through the whole process-from the referral to the final day of your child’s Wish. If you have any questions, please let me know!

We have designed this section to answer some frequently asked questions, as well as to provide you with some of the paperwork you may need to complete your Wish. I will snail mail each form to you, but if you want to beat the mail or need a new copy, you can print the form off, sign and return to our office. Beyond this, we ask that you fill out the basic profile so that I can have easy access to everything I may need to know about your family while planning the Wish.
